Diclofenac inhibits the COX enzymes and so reduces the prostaglandins that cause pain, fever and inflammation. Applied to the skin as a gel it acts mainly at the painful joint or muscle.
Paracetamol blocks the production of prostaglandins in the brain — the chemical messengers that create the sensation of pain and push the body's temperature set-point up. It relieves pain and brings fever down but has very little effect on inflammation.
